- Dorsal approach
The skin is incised on the dorsum of the proximal and middle phalanges, and the extensor tendon is split.
- Removal of articular cartilage
Using a narrow Langenbeck retractor to gently separate the articular surfaces, the cartilage is removed by currettage.

- Plate placement
The narrow 5 hole DCP is placed dorsolaterally to prevent interference with the extensor process of the distal phalanx.
